This item is very efficient for its price, can buy in tougher lanes to die less (also builds into Colossus/Fortitude). Makes vents and fruit heal you for more

Can be good into punchy heroes (especially Viscous) or just bought for burger boxing
Grit
Can buy this to "counterspell" burst in lane or to play aggro, or if you're getting reactive barrier

Easy upgrade from Close Quarters that adds a lot of damage, can be replaced by Weighted (sell Close Quarters) if you need debuff res and no slot
Duration Extender
Can imbue on Siphon Life instead if wanted
Superior Duration
Built from Duration Extender
Makes your active abilities and items a lot better. Siphon, Charge, Ult, Unstop, Metal Skin, Colossus, etc. Easier to land wallstuns especially on new map
Debuff Reducer
Debuff resist is just a very good stat in 99% of games
Crushing Fists
Built from Melee Charge
10% additive resist shred with every heavy melee is very strong. Massive damage upgrade.
!THIS IS THE VERY LAST ITEM YOU SHOULD UPGRADE IN A GAME! You want to keep Healing Booster's effect late game so this is a natural upgrade
Spirit Resilience
Teams tend to do more spirit damage than gun damage, so this is better on average
Metal Skin
Good against pure M1 damage (Haze, Venator gun+gutshot), doesn't help with melee damage
Bullet Resilience
This works against gun heroes that also do significant melee damage (Punch Venator, Drifter, Bebop) but you already own a lot of melee res in build and get equal bullet res from warp stone
Colossus
Built from Extra Health
Big healthbar, lots of resist, lots of melee damage, an HMC, and an AOE slow
Indomitable
Built from Reactive Barrier
Solid upgrade in some games due to it shaving 74s off your ult CD if procced twice during the cooldown.
Plated Armor
This item is better if you own bullet resist behind it, and is very strong against onhit effects.
Ethereal Shift
Strong item for teamfighting or stalling if caught out so your team can show up or CDs refresh
Juggernaut
Decent against slows and if your team doesn't have a Juggernaut yet its higher value
Divine Barrier
Pocket is annoying
Cursed Relic
Good late game, can guarantee your team kills One Guy or stop fed carries
Phantom Strike
Good to rush after Hunter's Aura sometimes. Good for fliers/carries, also defensively against M1s because disarm
Healbane
Sometimes you need antiheal in a game and this is the cheapest option for it
Weighted Shots
If you need more debuff res, can replace Point Blank if no slots
Berserker
The big damage stat stick, I don't think it's necessary and I want greens more often but can come up
Spirit Snatch
Can be good in 2 circumstances, you're either maxing Siphon or you need to reach a 1shot threshold on a hero and this is more damage
Scourge
Good in late teamfights, AOE % damage and resists, loses to Curse (or being kited) though
Witchmail
Good very rarely in the late game against spirit comps but I feel like other items keep you alive better
